Old bike, old and new problems

The Rusty Raleigh is on the blink again:
1. grinding noise from the steerer or thereabouts: obviously something needs fixed, what & how? I have not got a clue what type of stem etc. I have, except that it's a mid-90s Raleigh that looks like this:

I''ve had problems for a while with the shifters: downtube shifters, originally Shimano 300EX, the indexing for the front shifter went after a while so I found a replacement on ebay, this worked for a while then the same problem, so I splashed out on new ones (R400 I think), now 1 year later it's happening again - indexing is gone, & it's a pain on good old-fashioned friction.
Is there any way to repair this, or am I going to have to keep on shelling out a ridiculous £30 or so (for a £100 bike) on new shifters every year?

Grinding noise is most likely to be bearings. Remove stem and forks, take the headset apart, clean out old races and inspect - if they're worn then replace the headset. If not, just replace the ball-bearings (loose are best as they give you more surface area, although are tricker to install) with fresh grease.
As for the shifters... is something wearing out or do they just need cleaning and relubricating?0
