How are handlebars measured?

I am looking for some new bars for my Planet-x project, I have just been and measure the bars on my old bike and they measure 42cm outside to outside and 40cm centre to centre.

does this make them 40s or 42s?

Also how do you know which width bar to go for? Do 40cm bars make the steering very twitchy?

Any advice appreciated

some manufacturers do outside-outside, but they are a small minority.

Most do centre-centre.
