Gear Changing and Hydraulic Disc Brake Query

Hi guys,
I've recently purchased a 2nd hand GT Avalanche 1.0, and its my first MTB since I sold my custom trials bike 6 years ago, so it's my first experience with forks and hydraulic disc brakes. I've given her a pretty thorough service, and she rides beaitifully save for a couple of problems. I'd like to get back in to fixing things, so any advice on these issues would be much appreciated.
1. In the lowest gear, the rear derauiller (Shimano LX) seems to twitch ever so slightly - once off the largest sprocket it stops.
2.Changing the front gears (Shimano Deore) on an incline results in a massive clunk (especially moving from the middle chainring to the inner chain ring)
3. As much tweaking as I can do, it seems impossible to get the chain not rub (only slightly) the front derailluer. This only occurs at the extremes.
4. My rear disc brake (Tektro Auriga Comp) feels unresponsive, I've replaced the pads and given the disc a light sanding to improve the friction, and it feels ok when I'm not on bike, but when you're actually riding I cannot get the response of the front brake with my weight on it.
