Rear hub / disc c.3mm too far out - causing disc rubbing?

Since buying new, the rear disc on my '09 Cannondale Bad Boy 700 has been grazing the brake calliper on the side nearest to the seat stay (left as you look at it from the back), instead of sitting dead centre in the calliper.
The rear wheel comprises an M525 disc hub and Avid Juicy 3 disc / calliper. I've adjusted the calliper on its mounts (post mount to IS adapter) as far as possible, but it still won't sit in the centre of the calliper. The bike shop hasn't really been helpful - they took it in for a day but failed to fix the issue, claiming that the disc was bent (it isn't; I swapped it for another and had the same issue).
I can think of two solutions:
1. Grind back the IS mount by about 3mm (!) - is this the same as "facing" the mount?
2. Increase the width of the rear axle by adding a 3mm washer
Can anyone help or offer any advice?

Facing the mounts doesn't take that much off lol - so i wouldn't go via that route!
It is possible that the hub is not in the correct position on the axle, but you can't just add a washer - what you add from one side you must take from the other (inside the locknuts), and then the wheel will need redishing0
