Youre not tightening up the quick release lever enough.

You've not damaged your dropouts? And you are FULLY seating it in the dropouts correctly, right?

Just make sure both of the above are true!

Do you flip the lever open, twist the nut on the opposite side of the lever, then pull the lever shut, so that it's fairly tough to close?

If not this EXACT way, then you are operating the QR the wrong way.

true, but then there is also the end position of the lever.

the number of Qrs i have seen incorrectly fitted is scary.

Yup, like in supermarkets... They all seem to fit the forks on backwards. Probably because they think it looks cool, but in reality, it's REALLY dangerous!

I think i may have been tightening it the wrong way, i was opening the lever and turning it so the spring was closing then shutting it

the backwards fork is actually not that dangerous on those bikes, I have scared the crap out of a lot of Swedes by waving an allen key at them to get them off there bikes and sort the forks for them.

You can do it that way aswell. Doesn't matter, as the axle is connected to the lever end, so screws in the same way it would if you were turning the nut.

How tight is it to close the lever once you've wound it up?

Yeah, maybe on adult bikes, but for kids.. On forks with mad-short rake's... Makes for REAL quick steering!

    front Qr shown as reference.

with the lever showing the words open. wind it in so that you have a little it of grap on the nuts ( some times you need to turn the acorn nut as the lever end will not turn)

then close the lever fully so that it is right back and not touching anything. and you see the words closed if pressent.

the closing force should leave a good impression on the palm of your "little hands"

It's quite hard to over-tighten them. You'll know if you are.

If you absolutely cannot get it to sit right, and it keeps popping off, take to to yuor LBS, and they can firgure it out for you.
