Campagnolo Centaur Ultra Torque crankset failure

Hello,
About six months ago my LBS installed a 2009 Campagnolo Centaur ultra-torque chainset. I haven't got the tools, torque-wrench etc necessary for such a job, so I let them install it and then I just left it alone.
I was out for a ride today with a friend, and shortly after I set off, I got out of the saddle to accelerate. The left crank arm came off, and consequently I hit the deck.
Having rounded up all the bits and taken a good look at it, it looks as if the central bolt holding the two halves together has sheared in half or otherwise broken. The top cap of the bolt has come clean off (where it has the necessary torque levels engraved), and the rest of the bolt is showing a lot of torn metal. Generally, everything else looks fine, except the BB cups, which are pretty chewed up too. But I put that down to the crashing, rather than being a root cause.
Has anyone else experienced a failure like this? My first thought was that it sounds like a manufacturing defect, and therefore a warranty claim, or a faulty installation.

Take it back to the shop who installed it - Campagnolo has a 3 year warranty so you should be covered.Make mine an Italian, with Campagnolo on the side..0

I bought it from an online retailer and my LBS installed it; I assume it needs to go to the retailer, rather than the installer?
I'll give them the details of who installed it anyway.
