Chain ring prob?

Hello all.
Was out riding yest and developed a prob that managed to ping me over the bars. Just cleared a section of twisty single track and got up out of the saddle to push up a short steep climb that follows. gear ghange was fine preparing to climb. within 5 spins of the crank on the climb, the crank locked solid and i popped over the top of the bars. much to my brothers amusement. once i picked myself up i inspected bike. found the chain had not cleared the middle chain ring on the non drive side and wound up against the front mech. also the chain had slipped from first gear on the rear mech down to top gear. once i back pedalled it cleared. the was the case right through the ride. but only when on middle ring. once bike was cleaned off found a tooth has snapped on top gear of rear cassette. i suspect worn middle ring causing chain hang up. deore lx chainset fitted. lx front mech, xt rear mech, hg73 chain, pg 970 cassette, xtr shifters. always kept clean and well lubed. all parts around 10 months old. any more ideas on cause would be very helpful.
many thanks.

Worn/mismatched parts.

It's caused by the chainring teeth becoming hooked and not releasing the chain properly. Wet and/or muddy conditions can make it worse.0

was at bedgbury. some sections were ok. others were terrible. and the new section was pretty much un rideable cos of the slop. was servicing bike last weekend and noticed front chain rings were starting to "hook" a bit. i guess everything probably took its toll. i ended up with a face full of mud, and my brother ended up with sore ribs through laughter.0
