Help - Formula K18 Lever loose bolt

My Formula K18'S are great on my Meta 5.5 are great but recently the bolt that connects the lever arm to the body of the lever keeps shaking loose.
This means i am constantly having to push it back up in case it falls out - quite distracting when on a decent.
There is nothing obviously missing compared to the other lever and i am at a loss as to how to fix it.
Anyone else had this problem or that can offer a solution?

sounds like the grub screw has fallen out. back to the LBS for fixing.
there was a recall about this problem.
