Chain keeps jamming or coming off.

I rode a Spesh MTB for 2.5 years with Shimano cranks and gears and I only lost the chain twice. Now every time I go out on the Wilier with Camapag Centaur I lose the chain on the outside. The bike has been serviced and I can see that everything is in the correct position and the indexing is fine yet sometimes when changing to the large ring the chain falls off.
The inside of the derailer cage is no where near touching the chain when in the lowest gear. Could it be something I'm doing wrong and there is a certain change sequence with the rear and the front that it can't handle?
The other really annoying thing it does is when changing to the small ring the chain jams and I have to back pedal to free it. What would cause this? It's usually going up a hill which adds to the bad experience.
Any advice would be most welcome. Not only is it spoiling the ride its not doing the chain stay and the crank much good either.

Sorry if this is stating the obvious, but have you made sure that the chain limit screws are set correctly (on top of the front dérailleur)0

Sounds as though the gearing is all to hell
Have a look at Sheldon Brown's (www.sheldonbrown.com) website on gear adjustment and you should soon get things back on track

Is your front mech aligned correctly?
The outer edge of the front mech should be parallel with the big chainwheel and about 3mm above its teeth.0 -
sounds to me like the outer limit screw needs adjustment.0

Thanks everyone. I can't believe its come back from first service with something as basic as that but I have screwed it in slightly, need to test ride it. Also the rear mech wasn't quite moving out far enough causing a clicking noise in the highest gear. Still don't know why the chain locks up when changing to the small ring0
