Bent rear axle

I have a ridgeback speed bike which i use for getting to work. The original axle got bent after around a year (a QR axle). I paid for a new wheel at the LBS, the cones on which weren't properly tightened and that wheel wobbled around and after a couple of rides the axle was slightly bent too. They replaced this FOC and checked the cones and lock nuts. After around another 50 miles the cones were loose again and the axle slightly bent. I went to a different LBS (as i`m up at uni this time of year) and they tightened the cones and said just bent up with the bent axle until its knackered then replace.

This axle is now well and truely bent and am wondering what to do next. Can I just replace the axle? Its a cup and cone style hub on machine built £30 type wheel. Should I replace the wheel again? If so how can I stop it bloody bending. I have checked the frame alignment using the string tied round the rear dropouts and round the headset and all looks quiet alright.

If I am buying a new wheel I was considering going with a casette (have free hub right now) and getting a new cassette with less teeth. Will this be okay if I stick with the same number of cogs?
