slipping gears !

hi can anyone help me , i've got a kona cindercone and over the past week the gears have starting slipping when it is on the middle cog on the crank and the large cog on the wheel ! it slips out of the middle cog on the crank onto the small cog ! hope this makes sense ! any advice would be greatly appreciated !

Have you tried adjusting the front mech?
