I know at some point that I'll have to change to am external bearing BB. Is it just a case of taking one out and putting the new, external bearin BB, in? I suspect nothing is that easy.
I've added a signature to prove it is still possible.

yes but you may need the bb shell facing if it has not been done before.

how much difference does facing the frame make nick? how long should these bearings last anyway?

well shimano say that the frame HAS to be faced.

If the sides are not perpendicular to the axle axis and parallel to each other then wear will happen faster than it should. Some of the bearing makers use self aligning bearings but i have no idea if any of the external bb cups use these yet.
