Headset Service Advice

Do I just undo the hex bolt at the top of the stem to get to the bearings to give them a re-grease?

I'm getting a graunchy feeling when I turn the handlebars and also there is a bit of rusty water that occasionally seeps out of the bottom cup.

Any ideas?

    undo the stem clamp bolt(s) then the top cap and you should be able to pull it all apart. once inside, check to make sure everything is greased and clean. by the way, keep check on where everything goes for when you re assemble.
