Have you got the limit screws adjusted properly?

Is the mech damaged? I recently had to change my rear mech, due to similar symptoms to yours.

dont think mechs damaged.....limit screws set to cover both extremes ok...like i said its not on all, just maybe a couple are effected, when i alter the tension on these to correct it the effect is transferred so not always on same area red

Indexing on campag is set up by placing the chain on the 4th smalled sprocket, it needs to be perfectly alligned on this cog, to do this turn the cable adjuster clockwise to pull the pulley outwards, and turn it anti clockwise to pull the pulley inwards.

That might help, you need to do this as well as the outer and inner limit screws.
