Trouble with jumping gears

Hello all,
Proper newbie here,
bike Spec Rockhpper Comp with XTR rear derailler.
Riding back home from work and got to set of lights set off and the chain kept jumping up a gear made a really horrible grinding noise.
Got Zinn's book out and i either have brain freze from the ride or am just dense. probably the later but i cant work out what to do,
SO far i have tightned the cable, :idea: tightend and slackend off the B screw :idea: and adjusted the reach with the two screws on the deraile arm. :idea:
Now the problem is the chain trys to get from the small cog onto the second smallest cog with some skipping and clunking.
Anybody else had this problem or got any ideas i can try :?:

ok taking as the XT mentioned the spring works the normal way. pulling the mech to the small cog.
so you have one or 2 problems (or both) the stop is not allowing the mech to reach the bottom cog or the cable tension is too high stopping it reaching the bottom cog, Or both."Do not follow where the path may lead, Go instead where there is no path, and Leave a Trail."

On a different path, it was mentioned in MBUK when they were testing £500 hardtails that alot of new bikes seem to suffer from chain skip/slip for the first 30km's or so, they put it down to the grease the drive train is coated in from teh factory.0

Other thing to make sure if you do alot of off roading is to make sure your cables are clean. Watch the how to video on the main site.
