Sonic, have you ever fitted a self extracting crank bolt with the crank in place? Could be tricky for someone who (no disrespect) didn't know the crank wouldn't just pull off. You'd need to tighten the bolt until the cap was close enough to catch a crank thread and then progressively screw both up together. If they got out of step, they'd bind.

Not for you, but (again no disrespect), the OP didn't know that the crank wouldn't just fall off if the bolt was removed and then didn't realise that they had a self extracting bolt all along (although what I don't understand was why the crank didn't come off with the bolt!)
