This seems to be a perennial problem with my bike. It's a De Rosa, with a carbon steerer; I've got a new Pro expander plug thing which worked ok-ish most of the summer (super smooth French tarmac, some tightening still required) but now seems to loosen up after about half a mile of crap British street surface. Then the rest of my ride is clunky and the bike seems juddery every time I apply the front brake. (I ride carefully and generally try to avoid cobbles of that makes any difference...)
Any tips?

I experienced a similar problem recently with an FSA carbon aheadset bung I just couldn't get it tight enough to stop the headset creaking. Turns out the aluminium insert was moving inside the cap so I just couldn't get it tight enough - since changed to a regular alumunium one and it's OK. Can't believe you get such poor quality on a £60 headset. Agree about tightening up the stem bolts to hold the tension.Make mine an Italian, with Campagnolo on the side..0
