Exploding inner tube and a slow puncture !

2 seperate incidents recently :
Firstly, my brother has had 2 exploding inner tubes (sounded like a shotgun) - the inner tube that remained was shredded. I guess this could be due to something sharp inside the rim, but has anyone else had a similar experience and what caused it ? Interestingly though, the tubes exploded in different places .....
Secondly, I had a slow puncture on a rear tyre. Took the tube out and fixed the needle sized hole with a patch. Tyre inflated fine and was OK until I went out - lost half the pressure within 20 mins, I guess due to the additional weight - looks like the fix hasn't worked. Looking for recommended fixing kits, or am I better just replacing the inner tube ?

probs better just getting a new innet tube, are you sure you didnt nip the tube on the rim?0
