The guy in the garage has a problem, a 4 stroke problem

The guy in the garage next to the warehouse i work in was was trying to fix his bike during his lunch hour so i went over to have a look. It isnt a road bike its a claude butler something or other i think its a hybrid.
His problem which i dont know the answer to and neither did he is that when he pedals every four strokes he feels a jump in the right foot, then four strokes later the left foot. and so on. its always four strokes.
anyone got any ideas?
Its dont ALOT of miles and the chain was filthy, disgustingly dirty. I said i would have a think
