dirt in bearings?

A couple of months ago I got a ridgeback velocity for commuting to work. Ever since going for a long ride along the canal toepath when it started raining heavily and I got particularly muddy I've been feeling a bit of grinding from somewhere around the back wheel.
At first I thought it was something to do with the deraileur or the chain but I've just had a puncture (a sewing needle through both sides of the inner tube???) so took the wheel off, as i turned it I could still feel it grinding so I've taken the ball race apart to find at one side the grease had become a rusty gritty paste.
I'm just cleaning it out atm but going to have to get some grease before I put it back together but is there anything I can do to stop this happening again? And also how tight should the nuts be put back on around the ball race?
