rear gears

Been having a few problems shifting gears some times the chain wont move on to the next cog down near the middle rear cogs , I have gone to the end on the barrel adjustments which has help but I need more. I have never set up the rear gears before and not sure how to set them up to stop this problem ?

    What set up is it?

    Which way are you having issues with cable pull or cable release?
