Soooooooo, I spent a good deal of the weekend trying to fine tune my front and rear derailleurs. The rear one in particular was problematic. No matter what I do I cannot resolve the following: either I can't shift into the smallest cog with the other gears all perfect OR the third largest cog is off (lots of rattle / skipping) with all the others working fine. In the former case, looking over the rear derailleur shows that the pulley guide is a few mm too close to the spokes.

Things I have tried or looked at:
- barrel adjustment
- re-installed rear cassette (slight improvement; maybe the rear lockring wasn't tight enough)
- checked spacers on the rear cassette (6700 cassette, pretty sure the spacers are all the same width). The gaps between cogs appear uniform to the naked eye.
- Limit screws don't hinder movement

Things I haven't tried yet:
- cleaning the cables
- Checking the rear hangar is straight (not had any falls on it though)

Which of these do you think is more likely? Or have I missed something obvious?

    I've found that for 6700 the cables have to be lubed, the rear mech outer has to be longer than you expect (i.e. with a straight run of outer into the mech).

    Since I've done this, my Ultegra shifts very well. If this doesn't work then get the hanger checked.
