Slipping/moving cassette

I run an Ultegra 10sp on Mavic Kysiriums with the correct spacer at the hib side. Recently the cassette comes loose and rattle on uneven road surfaces, I dis-assemble and tighten lockring but after a couple of rides it's loose again. The cassette also sprockets move clockwise/anti-clockwise in the hub. The cassette is quite old, but the teeth are not worn.
a) Can you get a 'shim' so it tightens more?
b) Can worn cassettes start to 'move' on the hub before signs/evidence of wear on the teeth?
c) Is it anything to do with Shimano cassette on Mavic hub?

Sounds like your lockring's knackered, assuming you're torquing it up tight enough. Try getting a new lockring from your LBS and see if that solves it.0
