SPD-SL cleat shims and nuts

after a 45mile ride this weekend i noticed there was a bit of play when clipping in and out on my bike, and when i got home i noticed one of the nuts and the cleat shim had come away and vanished....! is this a regular occurrence to other riders? and do any spares exist or do i just need to buy a new set of cleats to cover.....?

I've never had a cleat bolt come loose. Though I do know two people that this has happened to.

Never had this problem either but i do check the tightness on all my kit frequently on both my bike and my cleats, the roads around me have some craters in that will shake you to death!!
