Dropping Chain

Over the past few weeks I have been dropping my chain when shifting from the 34t to the 50t. Until this recent occerance I've never dropped a chain when changing gear. I can save the chain by changing back down to the small ring while still riding but I've had a couple of near misses.
Would this be the cable being too tight or does the outermost screw need adjusting?
I use a Ultegra front derailuer

As it was previously OK I'd check that the derailleur alignment is still correct; sounds like it's had a knock.- - - - - - - - - -

I had this start happening out of the blue on my winter bike last year. I put it down to chain stretch making it just sloppy enough to hop over the big ring (as you describe) with an overzealous shift up.
Give the upper limit screw on the front mech a half turn in and it should stop it.0
