Dropping my chain while shifting up - help?

Hi all,
I have an irritating problem on my focus cayo. It's a compact with an Ultegra groupset. When I'm in the big ring and shifting up fairly quickly on the rear cassette (ie heading towards the 11t) I often lose the chain over the top of the big ring (it drops off the outside). It sometimes happens when shifting from the small ring to the big ring. I've tried tweaking the limit adjuster and I've checked that all of the teeth on the big ring are straight but so far nothing. I haven't been able to recreate the problem on the work stand.

i'd start with a really good look at the front mech...
is it bent/crooked?
get the shimano instructions and follow them from the start to ensure correct fitting and adjustment, if it's straght and set up correctly then the chain shouldn't be able to come off this easily
