Which model are you talking about?
I have the EA90 version and you're supposed to adjust it with the wheel in place.

It's pretty tricky though if you haven't got the newer preload adjusters which allow you to use a cone spanner. The older ones were smooth and were nigh on impossible to move with your finger tips unless they were off the bike.

I have the new bits and the spanner -

but no idea how to adjust, what it does, or when to do it

For all wheel hubs, adjust to get them sweet and then back off a bit - the action of the QR pulls them snug again. Bit of trial and error as it can vary hub to hub.
Make mine an Italian, with Campagnolo on the side..

Ahh, good stuff, the same wheels as myself.

I apply just enough preload (screw the adjuster in) until I cannot feel any movement at the hub/wheel interface using my fingertips when I waggle the wheel side to side.
I then back it off just a tiny fraction so that I'm not overloading the bearings from the side. Cartridges bearings aren't keen on too much side loading.

Seems to work for me though some wheel guru on here may be able to enlighten us to a better method :D

As for how often, with the new adjusters the preload should not need altering that often once you've set it. Just check for the 'waggle' at the hub occasionally to see if you need to take up any slack that might develop.

Sweet = bearings are smooth with no discernable play or roughness/grittiness.

ec90 with the plus and minus sign on the hub,it only turns about 1inch either way and dont seem that firm.I have to turn it max to stop sideways movement. If i back it off a little then there is movement.I find it turns easy on bike dont know if thats ok or not.
