chain problems

Can anyone offer any advice, when I'm out riding my chain cinks sometimes causing it to lock, my chain is still on the cassettes but a few of the links have doubled back on themselves and cause the cain to lock out!

Is it because the chain is now too slack, can it be repaired ot do I need a new chain?

    has it been lubed/cleaned?
