Bottom Bracket and Headset???

hey guys my bottom bracket seems to be making and odd clunk when i pedal, any ideas??
and my headset whenever i pedal uphill or stand up to pedal makes the oddest clicking noise/?? i have had it apart and put it back together and it stopped, but started again. any pointers much appreciated.
