the locknut outside the cone needs to be tightened onto the cone, if that's done it *should* stay tight

is the qr skewer nice and tight?
my bike - faster than god's and twice as shiny

When you tighten it all up does the axel still spin freely?
If as you say the cones keep coming loose which side is it happening on?
I would remoce the axel completely and tighten up the cassette side as you cant do it in the hub, this side may be loose and in need of the cone and locknut tighening together. Then put you axel back in and repeat for non drive side.

You need to tighten the cone agains the locknut to 'lock' it into position otherwise it'll just unscrew again. You could also try some medium Loctite if it's a persistent problem e.g. worn threads.
Make mine an Italian, with Campagnolo on the side..
