Rear dérailleur adjustments

I'm having some difficulty getting my rear derailleur set up correctly. Heres how I've been doing it - maybe I'm doing something wrong
Released the screw holding the cable so its slack
Set the high and low limiter screws
wound the barrel adjuster all the way out (and then forward a turn)
Retensioned the cable and tightened the holding screw
Set the indexing using the barrel adjuster
Now this is all fine, until I move the chain to one of the other big rings and then the indexing seems to go to pot.
anyone got an idea what I'm doing wrong?

that doesn't make sense though - If you wound it all the way in, you wouldn't have any travel left for putting more tension on the cable when you came to indexing.0

You wind the adjuster OUT to add tension to the cable.0

I must have bee winding it out then as it was definately releasing the tension on the cable.0

Small adjustments can have noticeable effects, as little as quarter turns.
Plus certain gear combos should not be used.0 -
if you start again with the cable, winding all tensioners in so no tension is on the cable and as suggested, One turn on the mech adjuster just for fine tuning and get the gears spot on, then front shifting shouldnt make and difference to the rears??
i thin ksupposed to det them up on middle cog or so i've heard, (never had issue with mine being set up on granny)Timmo.

ok - finaly found out what my problem was - hopefully this could help anyone who encounters the same problem:
from Sheldon Browns website ...If the rear indexing works properly when using the large chainwheel but not on the small chainwheel, or vice-versa, this is often a sign that the rear derailer hanger is bent.
