having a bit of trouble adjusting rear gears???

ok, I'm having a bit of gear trouble. they've not been changing properly. I can't get it to go onto the smallest cog and it's not gearing up properly either.
it gears down perfectly to the 8th cog but then won't fo onto the 9th.
so, when adjusting does it matter what gear I'm in and do I tighten or loosen the cable dial thingy (i'm not sure what its called. (the little thing near the gear levers you adjust))
