gear mech out of shape

Hi, firstly let me apologize in advance for the improper termanology ( i`m new to the sport) but I hoped someone could offer advice on the following, when i`m changing gear sometimes the rear gear changer mechanisam (shimano xt) slips from the dog leg shape and sits back causing the chain to jam or come off the front ring..........anyone!

Make sure it is in the gear hanger tightly (dog leg thing) and that the stops are adjusted properly.
