Fitting Disc Brakes To Old Frame

I think I probobly know the answer, but is it possible to fit disc brakes to a V brake style frame ?
I know forks are not a problem, but tis the rear.

the mounts for the rear brake caliper on on the non drive side seat stay (the bit of the back triangle from the seat post to the back axle.) 2 mount points about an inch apart.
another clue is the brake line mounts, also on the seat stay, to guide the brake line down.0 -
I think he means a frame with no mounts - you can fit a A2Z adaptor to some frame with flat dropouts.0
