Slight play in rear hub - new wheel?

Hi all
I know very little about the technical side of mountain bikes, so apologies if this is a pretty simple question, but.....
Ive had my bike for just over a year, and the rear hub seems to have become a bit loose. In other words, when the wheel is firmly locked into the frame, there is a small amount of play, almost like the axle is loose. This means there is a bit of juddering when i brake at low speeds. :?
Does this mean a new hub/wheel? or can you service bearings in hubs?
FYI, its a 2008 Trek 6500, with Deore wheels.
Cheers
