Any particular reason you need lock-ons? Decent slip-on grips can be a smaller diameter and provided you secure them properly you shouldn't have any problems.

Dave, the wife had a bad crash last winter when one of her grips slid on a very wet day in Cumbria. Shes been locked on ever since. Said I would try for locks if they are available.

Interestingly, I read in a review of ladies specific rigs in What Mountain Bike, that the Marin Juniper Trail comes with custom bars measuring 19.1mm at the grip so Nicklouse is right, they are not readily available. And yes, it seems no lock ons are available either.
