Stem clamp size help

Hi all
I'm about to order a new stem for my bike, and have a couple of queries.
1: Is there a standard size for the height of the stem clamp?
2: If I fit my new stem, with spacers, but there is still a small amount of head tube gap (for want of a better description) am I likely to run into problems or am I better off trimming the size of the head tube so that there is no gap at all? Looking at pictures on the internet I can see that some bikes have no head tube visible once the stem is installed, some have spacers, and some have bare head tube on display.

add more spacers. or you will not preload your bearings.

Spacers under the stem is no problem, or you can have no spacers under there if you want.
The pic Nick has posted perfectly explains the important bit which is making sure there is about 3mm from the top of your steerer to the top of the stem so you can preload the headset with the stem cap.Dave S0
